1.            Approve payment of invoices.

 

2.            Approve payment of Invoice No. 4899173 in the amount of $3,860.20 to URS Corporation for engineering services for Runway Lights, Airfield Signage and Marking Improvements project at Suwannee County Airport.  (FAA grant)

 

3.            Approve payment of Invoice No. 4892549 in the amount of $11,064.66 to URS Corporation for engineering services for Airport Master Plan Update and Airport Layout Drawing. (DOT/FAA grant)

 

4.            Approve payment of Invoice No. 11799 in the amount of $27,252.00 to Suwannee Glass, Inc. for replacement of windows and installation of new storefront at Suwannee County Coliseum.

 

5.            Approve payment of Invoice No. 62416 in the amount of $26,776.42 to Anderson Columbia Co., Inc. for work performed on 129th Road. (District 1 funds)

 

6.            Approve payment of Invoice No. 62417 in the amount of $13,349.61 to Anderson Columbia Co., Inc. for work performed on 149th Road. (DOT grant)

 

7.            Approve payment of Invoice No. 62665 in the amount of $6,650.15 to Anderson Columbia Co., Inc. for work performed on 92nd Terrace, 92nd Trail & 101st Drive. (District 4 funds)

 

8.            Approve payment of Invoice No. 62666 in the amount of $31,869.12 to Anderson Columbia Co., Inc. for work performed on 91st Road. (District 4 funds)

 

9.            Approval of Change Order No. 1 for McInnis Services for an increase in the amount of $2,263.00 for the installation extra bathroom door in Judges Chambers and additional floor and TV outlets for the Judicial Annex building.

 

10.        Approval of change to Grant No. C0061 with the Florida Department of Health, for an increase the amount of $5,000.00, to include a Special Events Vehicle for Fire Rescue.

 

11.        Reappointment of Richard Grillo to serve another term on the Tourist Development Council, retroactive to January 1, 2011.

 

12.        Reappointment of Charles Thomas to serve another term on the Tourist Development Council, retroactive to January 1, 2011.

 

13.        Reappointment of Dick Calvitt to serve another term on the Tourist Development Council, retroactive to January 1, 2010.

 

14.        Approval to add 2010 Ford Explorer (Vin# 1FMEU7DE4AUA04146), donated by the Lafayette Health Department, to the County’s automobile insurance policy.

 

15.        Approval of corrected Subordination Agreement for Teresa A. McWatters, SHIP Client.

 

16.        Approval of Subordination Agreement for William and Amber Blackmon, SHIP Clients.

 

17.        Approval of Annual Accounting & Retention Agreement with BlueCross BlueShield of Florida, Inc.

20.        At 9:00 a.m., or as soon thereafter as the matter can be heard, consider approval of Polling Place Lease Agreements with the Community Presbyterian Church and Advent Christian Village for the use of their facilities as polling precincts.  (Glenda Williams, Supervisor of Elections)

 

21.        At 9:00 a.m., or as soon thereafter as the matter can be heard, discuss, with possible Board action, server battery backup upgrades for Sheriff’s Office.  (Sheriff Tony Cameron)

 

22.        At 9:00 a.m., or as soon thereafter as the matter can be heard, discuss, with possible Board action, training funding for Public Safety Telecommunications (911 Dispatcher) Certifications.  (Sheriff Tony Cameron)

 

23.        At 9:00 a.m., or as soon thereafter as the matter can be heard, acceptance of Florida Division of Emergency Management Subgrant Award in the amount of $15,000.00 for Table Top Exercise Program and County Specific Training.  (Sheriff Tony Cameron)

 

24.        At 6:00 p.m., or as soon thereafter as the matter can be heard, hold a public hearing, to consider adoption of resolution approving proposed Special Permit for a Class I Landfill as a solid waste facility in an Agriculature-1 (A-1) zoning district.  (Ronald Meeks, Planning and Zoning Director)

28.        Authorization for partial insulation of Branford Public Works building by piggy-backing on the Sikes Enviroseal Insulation contract for the Suwannee County Coliseum.  (Tabled during November 15, 2011 Board meeting.)  (Randy Harris, Public Works Director)

 

29.        Authorization to bid partial air conditioning (replacement) for Branford Public Works Building.  (Tabled during November 15, 2011 Board meeting.)  (Randy Harris, Public Works Director)

 

30.        Consider recommendation of Bid Review Committee in awarding of bid for Agricultural Coliseum HVAC Upgrade to Shine Company, Inc. in the amount of $372,000.00, and authorization for Chairman to execute documentation after legal review. (Bid Solicitation No. 2011-31) (Joe Gerrity, County Coordinator)

 

31.        Discuss, with possible Board action, establishing procedures for the public hearing to be heard at 6:00 p.m. regarding the proposed Class I Landfill.  (Chairman Wesley Wainwright)

 

32.        Discuss, with possible Board action, the termination of Randy Harris as Public Works Director.  (Commissioner Jesse Caruthers)
