CONSENT:

1.             Approve payment of invoices.

 

2.             Approve payment of Invoice No. 0471354 in the amount of $6,453.60 to Arcadis US, Inc. for engineering services for Catalyst Site Water Use Permit.  (DEO/EDA grant)

 

3.             Approve payment of Invoice No. 12-0706 in the amount of $9,250.00 to Skeen Railroad Engineering for professional services for Catalyst Site. (DEO/EDA grant)

 

4.             Approve payment of Invoice No. 2 in the amount of $121,186.47 to Curt’s Construction for the Construction of 129th Road (Hughes Road).  (District 5 funds)

 

5.             Approval to advertise to fill vacant Machine Operator II position for Public Works Department.

 

6.             Authorization to advertise for bids for Sink Hole Remediation for 140th Street.

 

7.             Approve agreement with Frank Schulte, in the amount of $162.00 per month, for lease of Hangar “B-1” at Suwannee County Airport.

 

8.             Approval of contract between Judge Williams, Suwannee Valley Probation Services, Inc. and the Board of County Commissioners for Misdemeanor Probation Services.

 

9.             Declaration of broken, non-repairable equipment, as surplus for Live Oak Library, and remove from county inventory list.

 

10.         Approval of Small County Outreach Program (SCOP) agreement with the Florida Department of Transportation, in the amount of $1,251,246.00, for the widening and resurfacing of 216th Street (Market Road) from CR 49 to the Columbia County Line, and adoption of enabling resolution.  (Financial Project ID: 424333-1-58-01)

 

11.         Award bid for Construction of Wellborn Fire Station to McInnis Services, LLC in the amount of $244,896.00. (Bid Solicitation No. 2012-10) (Bids were opened during September 4, 2012 Board meeting.)

 

12.         Award bid for Security Cameras and Video Recording System for Suwannee County Airport to Simplex Grinnell in the amount of $37,081.00.  (Bid Solicitation No. 2012-14)  (Bids were opened during September 4, 2012 Board meeting.)

 

13.         Approval to transfer balance of $28,322.74 from 2010-2011 Budget to account designated to purchase elections voting equipment for Supervisor of Elections.

 

14.         Approval of annual contract with the Florida Department of Health in the amount of $154,564 for Operations of the Suwannee County Health Department for Fiscal Year 2012-2013.

 

15.         Approval to piggyback the Florida Division of Library and Information Services contract in the amount of $13,157.00 for the purchase of a Microfilm/Microfiche equipment reader and software equipment for the Suwannee River Regional Library.

 

16.         Approval of offer in the amount of $35,000.00 from William Powell for purchase of surplus Box Burner for Public Works Department.

 

17.         Approval of agreement with the North Central Florida Regional Planning Council in the amount of $7,421.00 for Annual Monitoring of Hazardous Waste Generators.

 

18.         Approval of agreement with the North Central Florida Regional Planning Council in the amount of $23,500.00 for Local Government Comprehensive Planning Services.

 

19.         Approval of Administrative Services Agreement with the North Central Florida Regional Planning Council in the amount of $58,000.00 (subject to CDBG grant) for Program Set-up, Program Implementation and Program Close-Out for the Small Cities Community Development Block Grant for Economic Development for Catalyst Site.

 

20.         Approval of Fiscal Year 2011 Federally-Funded Subgrant Agreement for Economic Development with the Florida Department of Economic Opportunity in the amount of $3,000,000 for a Small Cities Community Development Block Grant for Catalyst Site.

 

21.         Approval of for Community Development Block Grant Participating Party Agreement with Klausner Lumber One regarding infrastructure improvements for Catalyst Site.

23.         At 4:00 p.m., or as soon thereafter as the matter can be heard, open bids, for Guardrail Replacement at Intersection of CR 252 and 61st Road.  (Bid Solicitation No. 2012-16)  (Randy Harris, County Administrator)

 

24.         At 4:00 p.m., or as soon thereafter as the matter can be heard, open bids, for Galvanized Corrugated Metal Pipe for Public Works Department, and award bid.  (Bid Solicitation No. 2012-17)  (Randy Harris, County Administrator)

 

25.         At 4:00 p.m., or as soon thereafter as the matter can be heard, open bids, for Clearing & Grubbing and Earthwork (Catalyst Site Project), and award bid.  (Bid Solicitation No. 2012-21) (Originally Bid Solicitation Nos. 2012-18 and 2012-19 – Combined into one (1) bid.)  (Randy Harris, County Administrator)

 

26.         At 4:00 p.m., or as soon thereafter as the matter can be heard, open bids, for Sink Hole Remediation (Catalyst Site Project), and award bid.  (Bid Solicitation No. 2012-20)  (Randy Harris, County Administrator)

 

**At 5:15 p.m. Recess Regular Board Meeting to Hold Final Budget Hearing**

 

27.         At 7:00 p.m., or as soon thereafter as the matter can be heard, hold second of two public hearings, to consider the proposed Development Agreement, including the uses proposed for the property, the proposed building intensities and compliance of the Project with the Suwannee County Comprehensive Plan and Land Development Regulations.  (Randy Harris, County Administrator)

 

28.         At 7:00 p.m., or as soon thereafter as the matter can be heard, hold a public hearing, to consider adoption of resolution closing a portion of 50th Street.  (Ronald Meeks, Planning and Zoning Director)

 

29.         At 7:00 p.m., or as soon thereafter as the matter can be heard, hold first of two public hearings, to consider amendments to the Comprehensive Plan related to Solid Waste Facilities (Class I, Class III, and C&D Landfills), and approval to submit transmittal letter.  (CPA 12-02) (Ronald Meeks, Planning and Zoning Director)

 

30.         At 7:00 p.m., or as soon thereafter as the matter can be heard, hold first of two public hearing, to consider amendments to the Land Development Regulations related to Solid Waste Facilities (Class I, Class III, and C&D Landfills), and approval to submit transmittal letter.  (LDR 12-01)  (Ronald Meeks, Planning and Zoning Director)
